Here at American Family Care – Jacksonville, we’re dedicated to keeping children of all ages happy, healthy and safe.  We’re specialists in providing quality, personalized care for patients entering school and sports programs.  Known as a pre-participation physical examination (PPE) a sports or school physical is commonly required by state public health departments to ensure your child or teen is healthy and capable of participation without increased risk of illness or injury.  Children need to undergo these physicals and to submit up-to-date medical documentation before they can be eligible to play a sport, join a camp program, or enroll in school.

Our youth physical exams are customized to meet the needs of your child’s program and typically include:

  • Examination of medical history
  • General health screening
  • Measurement of height, weight, vision function, hearing function, & blood pressure
  • Administration of missing vaccines (if needed)
  • Referral to a primary care physician or specialist (if needed)

At American Family Care, we accept walk-in appointments for our full range of youth physicals.  We can provide your child’s school administrators or sports coach with the properly signed forms and answer any other questions you may have about your child’s health.
